Federal + state + 7.65% if you deposit through payroll. The example uses 22 + 6 + 7.65. California and New Jersey tax HSA deposits, so leave out the state rate there.

Medicare enrollment checklist

For you or a parent, before Dec 7, 2026. Changes take effect Jan 1, 2027.

  1. Read the Annual Notice of Change from the current plan. It lists what changes on Jan 1: premiums, copays, drug coverage, networks.
  2. List every drug with its dose and how often it's taken.
  3. List the doctors, hospitals and pharmacy that matter most, and check each plan's network for 2027.
  4. Get each plan's yearly drug cost from the Medicare Plan Finder.
  5. Ask which services and drugs need prior authorization under each plan.
  6. Compare a light year and a bad year, not just the premium. A $0-premium plan can cost more in a year with a hospital stay.
  7. Book free one-on-one counseling with your State Health Insurance Assistance Program (SHIP).
  8. Enroll by Dec 7, 2026. People already in Medicare Advantage get one more switch from Jan 1 to Mar 31, 2027.

2027 figures, confirmed with CMS: Part D out-of-pocket cap $2,400; Medicare Advantage in-network limit up to $9,850 ($14,800 combined for PPOs). The 2027 Part B premium had not been announced as of Oct 9, 2026 (2026: $202.90 a month).
